Sleep and appetite: what the science actually says (and how Makuro uses it)

Sleep tracking is one of the most requested features in every nutrition app, and it’s easy to see why: your watch already measures it, and it feels like it should matter for weight. It does — but not in the way most people assume, and the difference changes how an honest app should use it.
The effect is real, and it runs through appetite
When researchers restrict people’s sleep in controlled settings, they reliably eat more. In one carefully measured inpatient study, sleep-restricted participants consumed about 559 extra calories per day compared to when they were well-rested — and crucially, they did not move more or less to compensate (St-Onge et al., 2011).
The mechanism is hormonal. Short or fragmented sleep raises ghrelin (the hunger signal) and suppresses leptin (the fullness signal), which pushes appetite up and steers cravings toward energy-dense, carb-heavy food (Spiegel et al.). A systematic review of clinical trials reaches the same conclusion: cut sleep, and hunger, portion sizes, and total intake go up while fullness goes down.
The part most apps get wrong: your burn barely changes
Here’s the counterintuitive finding. You might expect that being tired makes you burn fewer calories — that a groggy day tanks your metabolism. Controlled trials don’t support that. When sleep is shortened under lab conditions, total daily energy expenditure stays essentially flat (Shechter et al., randomised crossover trial). The St-Onge study above found the same thing: activity energy expenditure didn’t budge. The weight gain from poor sleep was driven almost entirely by eating more, not by burning less.
This matters enormously for how a tracker should behave. Makuro’s whole method is to measure your energy expenditure from your intake and weight trend rather than guess it from a formula (that’s the methodology). If we let a night of bad sleep nudge your expenditure estimate up or down, we’d be injecting a signal the evidence says isn’t there — and corrupting the one number the app works hardest to get right.
How Makuro uses your sleep
So Makuro treats sleep as exactly what the science supports: a lens on your appetite, never an input to your calorie target.
- Turn on Import sleep (Me → Apple Health) and Makuro reads your nightly sleep from any app that writes to Apple Health — Apple Watch, Garmin, Oura, Samsung Health, and others.
- It pairs each night with the next day’s logged intake and shows you your own pattern: “On nights under 6.5h, you ate about 300 kcal more the next day,” or “Your intake held steady regardless of sleep.” It’s a correlation in your data, shown plainly — not a verdict, and not a hidden adjustment.
- Your expenditure estimate and calorie targets are untouched by it. They keep coming from what your logging and weight trend actually measure.
The practical takeaway is encouraging, too: because the damage is done through appetite, it’s catchable. Knowing that a short night tends to cost you a few hundred extra calories the next day is often enough to spot the craving for what it is. And in habitual short sleepers, simply extending sleep has been shown to lower calorie intake — one of the few “eat less without trying” levers that actually holds up.
